Frequently Asked Questions for clients and referrals

Please reach us at cdhs_sudshelpdesk@state.co.us  if you cannot find an answer to your question.

The SUDS Client ID is created by Salesforce and is used as a hyperlink to connect all information about the client.  There is a special field on client to enter a regional ID if you also give ID's to clients.

We now accept all entries of IA and ADRC into SUDS, we are not limited to only those over 60. They do not have to be a client to be recorded either, they could just be a referral (also known as Information and Assistance).

Yes! Just because a referral closes or does not become a client does not mean the information disappears.  It will continue to be stored in "Closed-Referrals" until a user reopens and changes it back to "in process".

You can enter clients without a referral if you wish to.  It is a region wide decision. If you decide as a region to not enter referrals, then the entire region will not have referrals. We can not specify down to a user. Please let the SUDS staff know if you are not participating in referrals or ADRC.

Yes, you can search by name, date of birth, address, regional ID or Peerplace Legacy ID.

Take a screenshot and email the helpdesk.  The referral goes off of first name, last name, date of birth and county so all of that information would have to match for it to link incorrectly.

Frequently Asked Questions for OC's, Dieticians, cm's

Please reach us at cdhs_sudshelpdesk@state.co.us  if you cannot find an answer to your question.

As per Policy and Procedure, one case note is required per one Nutrition Counseling session.

Smart Goals are

P&P section 411.77.H - Clients receiving in-home services (home delivered meals, personal care, homemaker) or care recipients through OAA/SFSS who score at high nutritional risk, or say yes to ‘Without wanting to, I have lost or gained 10 pounds in the last 6 months’, or ‘I have an illness or condition that made me change the kind and/or amount of food I eat’ shall be offered nutrition education and/or nutrition counseling they must be referred to the nutrition provider for nutrition education and/or nutrition counseling.

Case Note can be seen by anyone in the system with a SUDS user login.  Good examples of case notes are "bad address", "left voicemail", "no longer eligible for services", etc.  An ADRC case note is specific to options counseling and is only shared with other ADRC employees or Case Managers.

At midnight every night, SUDS batch creates units for every referral that you made.  This means you are no longer required to count how many referrals/Information and Assistance calls you did for the day.
